Proforma 1: Student course Evaluation Questionnaire will be filled by the student of each course before the time of ending semester.

Proforma 2: That proforma will be filled by the each faculty member of each course before the completion of course.

Proforma 3: The survey will be filled by the Graduating Students in last semester/ year before the award of degree.

Proforma 4: To be filled out by Master/ M.Phil/ Ph.D research Students on six monthly basis.

Proforma 5: To be filled by the each faculty members. The purpose of this assessment is that to assess faculty member’s satisfactions level, effectiveness of the program.

Proforma 6: The following information is required for each department in which a ph. D program is offered.

Proforma 7: The purpose of this survey is to obtain alumni input on the quality of education they received and the level of preparation they had at university. The purpose of this survey is to assess the quality of the academic program.

Proforma 8: The purpose of this survey is to obtain employers input on the quality of education university of Fatima Jinnah Women University is providing and to assess the quality of the academic program. The survey is with regard to University of Fatima Jinnah Women University graduates employed at your organization.

Proforma 9: A faculty members are required to provide information on this form. They can update the information on regular basis.

Proforma 10: The purpose of this information is to get feedback of students on regular basis.
